The Program of All-Inclusive Care for the Elderly (PACE) gives participants the ability to stay in their home while receiving essential services and maintaining independence and physical and emotional well-being.

PACE Your LIFE provides comprehensive medical and social services to adults 55 and older who reside in their home, requiring assistance and mostly to individuals who are eligible for Medicare and/or Medicaid benefits. When an individual enrolls in PACE Your LIFE, he or she will work with their own professional care team. This team of professionals assists in determining the package of services that will best meet our participant needs so that they can improve and/or maintain their health. This plan will also allow the participant to remain independent in the community for as long as possible. Services are provided primarily in the PACE Your LIFE Center, but also may be provided in the participant’s home or within the PACE Your LIFE community network.

I was depressed prior to enrolling with PACE Your Life. I enjoy mingling with other participants and helping them. PACE is a good place.

PACE Your LIFE also includes all other services that are available in your area and determined necessary by your team of health care professionals to improve and maintain your overall health


For most participants, the comprehensive service package enables them to remain in the community with a good quality of life rather than receive care in a nursing home.


Participants who choose to enroll in PACE Your LIFE agree to receive all their services through PACE Your LIFE. (excluding emergency services)* All participants maintain the right to disenroll at any time.

Prior to my husband's enrollment in PACE he was on a respite stay at an assisted living community in PA for 3 months. This was a very difficult time and arrangement for both of us. The most important concern was being able to safely support him at home in Delaware and provide opportunities for socialization and also support for me. PACE Your LIFE stood out from other options right from my first visit. The caring and compassionate staff were amazing and something I will never forget. When his driver picks him up in the morning she helps me to feel good about the whole day. I would recommend PACE for its inclusiveness and the feeling of holistic in its approach.
