PACE Your LIFE Expands Personalized Medication Support for Older Adults through ChorusRx Pharmacy Partnership
PACE Your LIFE, the Program of All-Inclusive Care for the Elderly serving Kent and Sussex counties, today announced its partnership with ChorusRx to expand personalized medication support for older adults with complex health needs.
PACE Your LIFE helps adults age 55 and older live safely at home and in their communities through one comprehensive program. Participants are supported by an interdisciplinary care team that includes primary care providers, nurses, social workers, and other professionals working together to support each participant’s health, independence, and quality of LIFE.
Through its partnership with ChorusRx, PACE Your LIFE is adding advanced clinical pharmacy expertise to this team-based model of care. ChorusRx clinical pharmacists will work with the PACE Your LIFE care team to review medication plans, identify potential medication-related risks, and provide more personalized support for participants who often manage multiple medications and complex health conditions.

For many older adults and caregivers, managing medications can be challenging. Medication changes, missed doses, side effects, and interactions between medications can affect safety, independence, and overall well-being. The partnership is designed to give PACE participants and care teams an added layer of support focused on medication safety, effectiveness, and coordination.
