MILFORD, Del. The new PACE Your Life center in the Milford Wellness Village looks to serve seniors in Sussex County with chronic health conditions that need support, allowing them to set the pace of their own treatment. “To be able to serve our members receiving Long Term Services and Supports in the community and coordinating their Medicare and Medicaid Services is the goal of DMMA and our PACE Organization,” Chief of Managed Care Operations in DHSS’ Division of Medicaid and Medical Assistance Kathleen Dougherty said.

“I think here this program addresses some gaps in services. It’s harder to find providers down here in the lower counties. Also, it’s a more rural area so transportation is really a challenge as well,” PACE Center Executive Director Lisa Bond said.

The center includes a primary care practice, day center, gym and even a salon.

U.S. Senator Tom Carper got a first look at the facility and says he looks forward to what this will mean for Delawareans. “It’s a great idea and its been tried in other places. We started this about 10 years ago in Northern Delaware and it works. This takes care of the central part of our state and the southern part of our state,” Senator Carper said.

The center also acts as a one-stop shop, providing an array of resources all in one location. “You get an entire interdisciplinary team to manage your care. So you will have a doctor, a clinic nurse, a home health nurse, a social worker, a physical and occupational therapist, registered dietitian, and activities coordinator. They work together to make a customized care plan,” Bond said.

The PACE program works with participants and their families to create the best treatment plan for the specific individual. Transportation services are also available to take patients to and from the center or even to their scheduled appointments with a physician.

Treatment doesn’t stop at the walls of the center, as the program also provides home health services. “They help with things like getting a bath, getting dressed, and meal preparation,” Bond said.

“On days that you want to stay and receive services right in your home, that is your option. So it’s always putting that care plan in the hands of the individuals. They’re directing their care plan,” Dougherty said.

The Executive Director tells 47ABC, they’ve already received 23 referrals of people interested in services.

Funding for the program is provided through Medicaid and Medicare, with services free of charge to those who qualify.

An open house is set for June 16th, from 11a.m.-2p.m.
